This form must be FAXED and SIGNED prior to returning wig, or your wig will be REFUSED!
Simply fill out the form how you want it remade and faxed to the number above!
Guarantee - Our goal is to satisfy, however, we do not issue refunds. What we do offer is the option to
repair (wig color, baby hair, cap and lace color etc.) or replace (too small or too large) if we made the
mistake. If
you selected the incorrect cap it is not returnable for another lace wig. Upon arrival we will
decide if the unit is a repair or a remake, depending on the reason for return. The wig cannot be
altered by you in anyway upon arrival or this agreement will be null and void! Returned packages must
be accompanied by a copy of the original invoice and must be returned in the original condition .
Product must be in the original packaging with all netting and tags in place. We will not accept
merchandise that has been altered (brushed, combed, picked, cut, or washed) in any way.
Merchandise packaging cannot be marked, taped or otherwise altered. Returns for all reorders must
be sent back no longer than 5 days after receiving merchandise. A Return Authorization Number(RA#)
is required on all returns. Call or email
Sales@MyLaceFrontWig.com for all authorized returns! Write
number clearly on the outside of shipping label Ex. RA103. All merchandise not meeting the above
criteria will be refused or returned to the customer at customers own expense. There will be no more
exchanges accepted after this return and remaking the unit the second time.

when returning.
If it is lost in route before delivery, we are not responsible.
